The employee manually pushed the roll onto the slat conveyor, however the roll continued on, striking a barrier and causing the roll to bounce back to the slat conveyor. The roll then rolled back to the barrier, striking the employee in the left arm and fracturing the ulna.

An employee was working with hot aluminum sulfate. During the reaction portion of the aluminum sulfate process, the agitator was turned on, causing the aluminum to boil too fast and crack the lid over the sulfate. The hot aluminum sulfate spilled onto the employee, causing second degree burns to the foot and the back of the right arm.

An employee was cleaning up hot pulp that had spilled on the floor (approximately 6-inches deep). The employee's rubber boots became stuck in the pulp. As he tried freeing himself, his left foot came out of his boot. His unprotected foot landed in the pulp, causing second degree burns up to his ankle.
